The use of the anterolateral thigh flap to reconstruct extremity defects in a paediatric population

We report a series of four ALT flaps performed in children
over a 12-month period. Three patients had tissue defects resulting from trauma, while another had break down of the previous
wound following amputation. We experienced an overall flap success rate of 100% and had only minor complications of partial
wound dehiscence in one patient and superficial infection in one patient. The function and cosmetic outcome of these patients
at review was excellent. Despite the technical difficulties associated with microsurgery in this age group, the results of
this small study suggests the versatility of the ALT flap in adults may be equivalent to a paediatric population. The psychological
impact of any free tissue transfer on both children and their parents should not be underestimated.

Going up DIEAP creek—Coventry and Warwickshire Plastic Surgery Department’s experience with lifeboats and paddles

Abstract  The deep inferior epigastric artery perforator (DIEAP) flap is arguably the gold standard for autologous breast reconstruction.
Despite its manifold advantages, potential flap failure, often secondary to venous congestion, is a devastating possible complication.
An appropriate salvage procedure may avoid DIEAP flap loss. To identify, describe and analyse DIEAP flap salvage procedures
carried out by the senior author (and Coventry and Warwickshire Hospital Plastic Surgery Clinical lead) who has a special

Cosmetic surgery in the NHS: Applying local and national guidelines

Summary: There is no worldwide consensus, as to how healthcare should be funded, in a modern society. Limited resources in the UK, have led to restrictions on cosmetic surgery in the NHS. Guidelines governing access to cosmetic surgery have been formulated. A retrospective audit has been undertaken, to assess adherence to local and national guidelines, in an NHS trust. Ninety-nine casenotes were reviewed over 1 year. Data on complications were collected. Compliance to local guidelines was 44% and to national guidelines was 22%. Individual and ethnic aspects of preoperative planning for posttraumatic rhinoplasty

Abstract  The aim of this study was to compare the aesthetic results of post traumatic rhinoplasty among Europeans with populations
of healthy Caucasians described in earlier reports, and to collate correct parameters of nasal shape in healthy representatives
of both sexes and various races for improvement in aesthetic results of surgery. 3-D scanning of the face was performed in
54 patients after posttraumatic septorhinoplasty. Analysis of 3D model was based on two indices of the nasal proportions and
four angles of the region from 18 anthropological points. Parameters of the nasal shape in addition to gender were compared

Epidemiology of plastic surgery trauma in people with associated drug and alcohol dependence: developing guidelines for optimal treatment

We describe the challenges and
epidemiology of injuries in this group and suggest an algorithm for optimal management. A retrospective study was carried
out of medical records of people with drug and alcohol problems treated in the plastic surgery unit between January 2004 and
December 2007 inclusive is presented. Exclusion criteria: (1) elective admissions, (2) previous, but not current drug or alcohol
use, (3) a short history of excessive alcohol intake, (4) isolated burn injuries, and (5) no notes available. Injuries were
sustained by 77 people. Males (64) were more commonly referred than females (13), with a mean age of 37 years (20–65) at presentation.
