From simple interrupted to complex spiral: A systematic review of various suture techniques for microvascular anastomoses

Monday, November 29th, 2010

AbstractAccomplishing successful microvascular anastomoses is undoubtedly one of the most critical steps in performing free tissue transfer. However, the ideal technique has often been a subject of debate. Therefore, our objective was to review the current literature in an attempt to find objective evidence supporting the superiority of one particular technique. A PubMed and OVID on?line search was performed in November 2007 using the following keywords: microvascular anastomoses, microsurgical anastomosis, continuous suture, interrupted suture, mattress suture, and sleeve anastomosis. Our literature review found no difference in short? and/or long?term patency rates between the six main published techniques, which includes continuous suture, interrupted suture, locking continuous, c…

Wide Topical Negative Pressure Wound Dressing Treatment for Patients Undergoing Abdominal Dermolipectomy Following Massive Weight Loss

Monday, November 29th, 2010

Abstract  Postbariatric plastic surgery is considered to be a high-risk procedure, which entails such frequent minor complications as
postoperative seroma, bleeding and wound dehiscence. These occur with a high incidence, especially, following postbariatric
abdominal dermolipectomy. In order to reduce these complication rates, a new type of dressing with wide abdominal topical
negative pressure (TNP) application was applied. We performed abdominal dermolipectomy in 23 obese patients. The average body
mass index was 32.8 kg/m2, and the median age of the patients was 42.9 years. Ten patients received conventional standard dressings (control group
I), whereas the other 13 patients received a wide TNP dressing including the ventral and lateral trunk (negative pressure

Achieving autologous breast reconstruction for breast cancer patients in the setting of post-mastectomy radiotherapy

Sunday, November 28th, 2010

Conclusions and implications for cancer survivors  Both immediate and delayed breast reconstruction are safe modes of treatment, however patients that are expected to require
adjuvant radiotherapy may benefit from delaying reconstruction until completion of oncologic treatment.

“Upside-Down” Augmentation Mastopexy

Sunday, November 28th, 2010

Conclusions  Natural shape, stable position, and short scars are the main advantages of the upside-down technique. The typical breast flatness
after periareolar access is best corrected by the last-generation dual-cohesiveness anatomic implants, which the author strongly
recommends to obtain the best results with this technique. The Body Uneasiness Test (BUT) study showed 100% improvement of
patient discomfort.

Effect of the Sequence of Lateral Osteotomy and Hump Removal on the Aesthetic Outcome

Sunday, November 28th, 2010

Conclusions  The pattern of fractures after perforations of the lateral wall seems to be more regular if the perforations are done before
the removal of bigger humps. In noses with small humps or no hump, no difference is seen regarding the sequence of the perforations
in relation to hump removal.
